Bronson MyChart or Bronson patient portal is an Electronic version of patients’ health data and all patients can access their Electronic Health records online by logging in to their MyChart account anytime anywhere as per their convenience. 2. Type in your username and password in the fields provided.

Forgot Password

• Visit the Bronson MyChart login page and click the Forgot Password link.
• You will see the password recovery page on the next screen.
• Enter your Username, SSN and date of birth, and click the Next button.
• If you have an email address and phone number on file then you will receive a six-digit one-time code to verify your account.
• Enter this code and verify your account.
• After the verification, you will get an option to create a new password.

Forgot User Name

• Visit the Bronson MyChart login page and click the Forgot Username link.
• You will be redirected to a username recovery page.
• Enter your first name, last name, SSN, and DOB.
• Click the Submit button.
• You will soon receive an email from mychart@ohsu.edu on your registered email ID including your Username.

How to Register for New Bronson Mychart Account Online

1. Visit this link on your device in an internet browser.
2. Click on the “ACTIVATE ACCOUNT” button to be redirected here: https://mychart.bronsonhealth.com/mychart/accesscheck.asp.
3. Type in your Activation Code in the fields provided.
4. Type in the Last Four Digits of you Social Security Number in the fields provided.
5. Input your Date of Birth in the fields provided.
6. Click on the “Next” button.
7. Proceed to move forward and provide all the required information to complete the registration for a new online account.
